The PRI assesses a patient\u2019s <strong>current medical, physical, and cognitive status<\/strong>. The Screen <strong>determines the patient&#8217;s ability to return to the community<\/strong>&nbsp;or qualify for long term placement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Reasons for getting a PRI and Screen include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Determine what level of care is needed for a patient.&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Help families understand what their options for living arrangements may be.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Educate families about sources of payment and resources available.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Assist them to live as independently as possible.&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Required by NYS prior to admission to any long-term care facility, otherwise known as a skilled nursing facility or nursing home and is valid for 30 days.<\/li>\n\n\n\n<li>One of the first steps in applying for Medicaid<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"has-text-align-center wp-block-paragraph\">When you are determining which level of care your senior loved one needs, our Happier at Home team can help you.
