Senior Care Planning: Patient Review Instrument and Screen (PRI)

A Patient Review Instrument, known as a PRI, and Screen can be done in any setting such as a home, Independent Living and Assisted Living facilities, among others.  The only individuals that can complete this assessment include RNs that are certified in the PRI and Screen.  Our certified nurse will do the PRI on a fee-for-service basis. The PRI assesses a patient’s current medical, physical, and cognitive status. The Screen determines the patient’s ability to return to the community or qualify for long term placement.

Reasons for getting a PRI and Screen Assessment

  • A Patient Review Instrument (PRI) and Screen Assessment is an essential process, especially when planning for long-term care. Here’s why it’s so important:
    • Assessing the Level of Care Required: The PRI helps determine the level of care a patient needs based on their current health status. This assessment ensures that the individual receives appropriate care, whether it’s for rehabilitation, assisted living, or a skilled nursing facility. It also offers a more personalized care plan based on the patient’s medical and physical needs.
    • Guiding Families Through Complex Decisions: Deciding on future living arrangements for a loved one can be overwhelming. The PRI and Screen Assessment provide families with a clearer understanding of the available care options. Whether it’s transitioning to a nursing home or considering home care alternatives, this process helps families make informed decisions tailored to the patient’s well-being.
    • Providing Financial Education and Resource Options: A major aspect of planning for long-term care is understanding how to fund it. The assessment process educates families on potential payment sources like Medicaid, private insurance, and other available resources, helping to ease the financial burden and clarify what programs they may qualify for.
    • Promoting Independence Wherever Possible: The goal of any care plan is to help the patient maintain as much independence as their condition allows. By evaluating their needs through the PRI, the assessment process strives to enable them to live independently for as long as possible, in an environment that best suits their capabilities.
    • Meeting New York State Requirements: In New York State, a PRI and Screen Assessment is required before any admission into a long-term care facility, such as a skilled nursing facility or nursing home. This assessment is only valid for 30 days, meaning it must be kept up-to-date to ensure accurate care decisions.
    • A Crucial Step for Medicaid Application: For those seeking Medicaid assistance for long-term care, the PRI and Screen Assessment are among the first steps in the application process. Without this step, applying for Medicaid benefits for nursing home care would be incomplete.
